· Have never taken the CPA
Examination
· Have taken the CPA
Examination in Alabama
· Have taken the CPA Examination in another jurisdiction
The package contains the application form, instructions, fee information,

applicable Board Rules. All materials should be read carefully as many
items have changed since the paper-and-pencil exam format.

Change of Information Form
This on-line submission form can be used by test
candidates who need to provide updated information to the Board.
This on-line form cannot be used (1) to submit a change of name,
which requires submission of documentation, or (2) to apply to take the
Uniform CPA Examination.
